About Us

Heal at Home is RN-owned and operated…

Heal at Home, LLC was founded by Courtney Hogenson, a Registered Nurse with a background in UCLA’s Cardio-thoracic Intensive Care Unit. Through her work as an RN, she often noticed a gap in the care of patients leaving the hospital. Many discharged patients were barely well enough to leave the hospital, but not well enough to care for themselves at home. After successfully coordinating the care for a friend’s mother post-surgery, she decided to utilize her knowledge and expertise to help others navigate today’s complicated healthcare, housing and home care options.

About our Geriatric Care Management Services…

We realize the importance for families to have a medical representative or patient liaison advocating on a patient’s behalf when in a health crisis or simply planning for the future.  Today’s healthcare system can be complicated and tricky, especially without the medical expertise and advocacy of a nurse or doctor.  Not only do we understand the continuum of care, we can also translate a medical diagnosis, medications, side effects, diagnostic tests, treatments and thus behave as a Nurse Care Manager.  You can rely on us for an ongoing basis or sporadically as needed.  Whether you have the need for a  Nurse, Geriatric Care Manager, Patient Advocate, Caregiver, or finding the perfect Senior Community such as an Assisted Living or Alzheimer’s/Dementia Care, we are your go-to source.  We share our database of trusted advisors in all things senior-related, whether it’s a medical specialist, elder care attorney, financial planner, moving services, real estate, short-term rehabilitation, transportation, hospice and much much more….

Courtney is a proud member of the Aging Life Care Association and participates locally and nationally in this prestigious and well-respected organization. She is honored to hold the title of Advanced Professional Aging Life Care Manager and has earned the the designation of Certified Care Manager (CMC).

Our Registered Nurses/Geriatric Care Managers know, understand, and love working with seniors and their families.  As a teenager, our owner Courtney helped care for her grandparents in her parents’ home.  Both of her grandfathers had Alzheimer’s disease.  And both grandmothers suffered from dementia.  All but one of her grandparents were cared for in her parents’ home while she lived there.  She grew closest to “Pa” her father’s father who had advanced stage Alzheimer’s and required 24/7 care while she attended high school.  “Pa” moved into the guest room next to her and became a friend to her and her friends.  Everyone grew to know and love “Pa” who was often the highlight of school and family events.  After several years of caring for “Pa” and his Alzheimer’s worsening, he was accepted into the VA’s specialized Alzheimer’s Care unit.  Through caring for her own grandparents, Courtney developed an early awareness, appreciation and understanding of what all family members experience when seeking help for their loved ones.

She has participated in the Southern California Alzheimer’s Association as a fundraiser and was a member of the Young Professional’s Commmittee (YPC).  She joined the Alzheimer’s Association Speaker’s Bureau in order to educate the community on the risks, signs, treatment, and future for Alzheimer’s Disease and how it affects millions of people worldwide.
